I think the woman in this one is not so much the average “dumb, naive wife/girlfriend turned into consensual cock slut” that you see in all ntr and is just an unfaithful wife, which for japan is considerably rare. She might still be borderline retarded (as most female protagonists in hentai are) and still got tricked by the “doctor” but it doesn’t look like it was mostly contrived or unbelievable.
